Our Message To Your Doctor
Patient “John Doe” has requested the home visit laboratory services of Bio-Care Services Inc.
If you wish to order a home visit for laboratory services, please fill out the attached requested form and fax it back to the numbers listed.
I Have A Prescription At Home
If your doctor has given you a prescription or lab request form, contact Bio-Care to schedule a home visit.
Bio-Care will schedule the home visit and instructions will be left with the technician to get a copy of the prescription upon arrival at the patient’s home.
